Send us an Enquiry
At very first stage of enquiry, We ensure you to get initial response within 24 to 48 hour. As per the information provided by you in our referral form we assess whether we can meet your needs or not.
Finalise Support
Once you confirm to take next step to get requested support with Easy Life Home Care, we will request further information including:
- your NDIS Plan
- any Health and Behaviour Support Plans and
- additional details relevant to any specific support services required.
Sign Service Agreement
We will issue a Service Agreement and schedule of support for services to be delivered, the start and end date of services and the frequency and cost of services.
The welcome pack also includes Consent form, Client hand book, Client Charter, Privacy policy, Feedback and Complaint form.
Once the Service Agreement and other supporting documents signed, services could commence as per requested schedule.
Commencement of Service
Easy Life Home Care will always endeavour to match support worker best suited to your support needs and help you to work towards your goals. We always take feedback after the first service.
